I have no reference other than instructions for completion.  Does it now have the contract number on it in the appropriate block?  If so you might be okay but it still might be good to pose your question to the CO.

Yes, the contract number is listed on the form. Good point. It will probably won't cause any audit issues since the contract number is listed. Thank you!
