When would you use the SF 1414 vs the SF 1415 when needing Consent of Surety for modifications over $50K?

circumstances for use are described as follows:
1414- FAR 28.106-5

1415- FAR 28.106-3

28.106-5 application appears to be for modifications and consent. 28.106-3 appears to be for consent but not for modifications.

Therefore, if you need consent and it applies to a modification, Form 1414 seems to apply.
